Director Case Management
Position Summary
The Director of Case Management provides strategic and operational leadership for all hospital-based case management functions. This role is responsible for directing, coordinating, and evaluating departmental activities to ensure effective care coordination, discharge planning, regulatory compliance, and optimal utilization of clinical resources.
The Director collaborates closely with interdisciplinary teams, hospital leadership, and external partners to ensure continuity of care and achievement of organizational quality, throughput, and financial objectives. This position requires executive presence, strong communication and presentation skills, and a proven ability to lead teams through change while delivering measurable outcomes.
As a subject-matter expert in case management, the Director drives performance excellence through trust-building, team development, and data-driven decision-making. The role requires a self-directed leader who can manage multiple priorities, work independently, and navigate complex situations with professionalism, tact, and diplomacy.

Key Responsibilities
- Provide overall leadership and oversight of daily hospital case management operations.
- Direct and evaluate departmental operations, including case management models, staffing plans (skill mix and FTEs), onboarding, competencies, and use of information technology.
- Ensure compliance with organizational policies, regulatory requirements, and applicable standards of practice.
- Oversee core case management functions, including care coordination and discharge planning, ensuring timely, high-quality, and well-documented processes.
- Allocate resources effectively to meet productivity, quality, and patient throughput goals.
- Collaborate with executive leadership and case management leadership partners to achieve established performance, quality, and financial objectives.
- Analyze operational and performance data to identify trends, assess outcomes, and implement sustainable performance improvement initiatives.
- Establish and maintain collaborative working relationships with key stakeholders, including medical staff, nursing leadership, ancillary services, compliance, legal, and ethics teams.
- Lead interdisciplinary engagement to support continuity of care and effective transitions across the care continuum.

Qualifications
Education
- For RN Bachelor’s degree – Required
- For LCSW or LMSW - Master's - Required
- Master’s degree in Nursing, Health Administration, Business Administration, or related field – Preferred
Licensure
- Current, unrestricted licensure as one of the following – Required:
- Registered Nurse (RN), Licensed Clinical Social Worker (LCSW), or Licensed Master Social Worker (LMSW)
Experience
- Minimum of 3 years of recent acute care hospital case management experience – Required
- Minimum of 2 years of leadership or management experience in case management – Required
