Job Overview

We are seeking a dedicated and compassionate Registered Nurse - Field UAS Assessments to provide essential home healthcare services in the New York Metropolitan area. Duties

  • Conduct comprehensive UAS-NY assessments to determine eligibility and care needs, ensuring our patients receive the best community-based long-term care services.
  • Collaborate with interdisciplinary teams to promote our PACE program, educating potential participants and referral sources about the benefits of our services.
  • Review and maintain accurate documentation in EMR systems, while addressing any significant changes in patient health.
  • Engage in quality improvement initiatives to enhance patient care and outcomes, while adhering to CMS/PACE guidelines.

Qualifications

  • Current nursing license in the state of practice.
  • Experience in medical-surgical nursing or working within a hospital environment is preferred.
  • Familiarity with emergency medicine protocols and practices is advantageous.
  • Strong knowledge of anatomy and aseptic techniques is essential.
  • Ability to work effectively with individuals with disabilities, demonstrating empathy and patience.
  • Excellent communication skills, both verbal and written, for effective patient interaction and documentation.
  • Ability to work independently while adhering to established protocols and guidelines.

Pay Range:

  • $100 - $160 per assessment
