Job Overview
We are seeking a dedicated and compassionate Registered Nurse - Field UAS Assessments to provide essential home healthcare services in the New York Metropolitan area. Duties
- Conduct comprehensive UAS-NY assessments to determine eligibility and care needs, ensuring our patients receive the best community-based long-term care services.
- Collaborate with interdisciplinary teams to promote our PACE program, educating potential participants and referral sources about the benefits of our services.
- Review and maintain accurate documentation in EMR systems, while addressing any significant changes in patient health.
- Engage in quality improvement initiatives to enhance patient care and outcomes, while adhering to CMS/PACE guidelines.
Qualifications
- Current nursing license in the state of practice.
- Experience in medical-surgical nursing or working within a hospital environment is preferred.
- Familiarity with emergency medicine protocols and practices is advantageous.
- Strong knowledge of anatomy and aseptic techniques is essential.
- Ability to work effectively with individuals with disabilities, demonstrating empathy and patience.
- Excellent communication skills, both verbal and written, for effective patient interaction and documentation.
- Ability to work independently while adhering to established protocols and guidelines.
Pay Range:
- $100 - $160 per assessment
