👩⚕️ Position Overview: MDS Coordinator (RN)
Under the direction of the Director of Nursing (DON), the Resident Assessment Coordinator (RAC) leads the interdisciplinary assessment process for assigned residents. This role is essential to ensuring resident-centered, compliant, and holistic care planning in the post-acute and long-term care setting.
The ideal candidate is an experienced RN with strong assessment skills, exceptional documentation abilities, and a sharp eye for detail. This is an administrative nursing role that blends clinical expertise, leadership, and education.
✅ Key Responsibilities:
- Coordinate the interdisciplinary resident assessment process
- Ensure accurate and timely completion of MDS assessments in accordance with company, state, and federal guidelines
- Lead the development and ongoing updates of Comprehensive Care Plans
- Monitor residents to ensure care plans are followed and updated as needed
- Identify and report care plan variances to the DON and Care Plan Team
- Serve as a clinical and regulatory resource for the interdisciplinary team

🧾 Qualifications:
- Current RN license in good standing (state-required)
- Knowledge of general nursing standards of care and medical practices
- Strong analytical, critical thinking, and deductive reasoning skills
- Excellent computer skills and understanding of electronic medical records (EMR)
- Highly organized, detail-oriented, and self-directed
