The RN/LPN is a Licensed Nurse who functions both as a supervisor and manager to provide quality care and services to a specified group of residents. This individual is expected to assume the responsibility for his/her health care delivery and to supervise the work of other professional employees for the Unit(s) assigned. This individual is responsible for coordinating all aspects of resident care fostering both interdepartmental and intradepartmental cooperation. He/She communicates pertinent information to Administration, Physicians, Families, Consultants, and other service providers.
Qualifications:
- Dedicated to providing high quality care.
- Graduate of an approved school of nursing
- Current Pennsylvania licensure as a Registered Nurse or Practical Nurse.
- Demonstration of leadership ability.
- Displays caring attitude towards residents.
- Works well with people.
- Show enthusiasm and motivation.
- Demonstrates the ability to verbally communicate resident status/changes in condition.
- Demonstrates the ability to complete written communication required by State/Federal Regulatory Guidelines and Facility Policies.
Essential Duties and Responsibilities
- Determines Resident needs so that nursing care can be planned, implemented and evaluated.
- Monitors the Provision of Care on a daily basis.
- Delegates Nursing Care/other assignments to appropriate team member based on Resident needs/staff abilities.
- Communicates with Physicians regarding Resident’s care and condition. May round with Physician as time permits.
- Assures all documentation of care is complete and accurate.
- Works closely with families to facilitate problem solving. Maintains excellent communications to make them aware of changes in status, hospital admission, planned diagnostic testing and falls.
